1) Yoga Liability

I understand that yoga includes physical, athletic movements. As with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to and honor my body, discontinue the activity, and ask for support from the instructor. I will continue to breathe easily. I assume full responsibility for any and all damages, which may incur through participation.

Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. By signing, I affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness program. In addition, I will make the instructor aware of any medical conditions or physical limitations before class. If I am pregnant, become pregnant or I am post-natal or post-surgical, my signature verifies that I have my physician's approval to participate. I also affirm that I alone am responsible to decide whether to practice yoga and participation is at my own risk. I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter against the instructor, Alyssa Kuzins.

I have read, fully understand, and agree to the above terms of this Liability Waiver Agreement. I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional release of all liability to the greatest extent allowed by law in the State of Pennsylvania.

2. Coaching Liability

• I understand that the life coaching services I will be receiving from Alyssa Kuzins are not offered as a substitute for mental health care. I also understand that my coach, Alyssa Kuzins, is not acting as a psychotherapist, and does not purport to offer mental health care.
• I understand that Alyssa Kuzins will maintain the confidentiality of our communications only to the extent defined by the laws of the states in which each of us resides.
• I understand and agree that I am fully responsible for my well being during any coaching calls, and subsequently, including my choices and decisions.
• I understand that all comments and ideas offered by my coach are solely for the purpose of aiding me in achieving my defined goals. I have the ability to give my informed consent, and hereby give such consent to my coach to assist me in achieving such goals.
• I hereby release, waive, acquit and forever discharge Alyssa Kuzins, their agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages I may claim to have or that I may have arising out of actions, omissions, or commissions taken by myself as a result of the advice given by or otherwise resulting from the coaching relationship contemplated hereunder. I further declare and represent that no promise, inducement or agreement not herein expressed has been made to me to enter into this release. The release made pursuant to this paragraph shall bind my heirs, executors, personal representatives, successors, assigns, and agents.

I have read the statements above and I understand and agree with the points contained therein.

3. Religion

I understand that Alyssa Kuzins is not attempting to support a religion or influence my beliefs religiously.
